What is a new relative has parents, but the parents aren't in
family_relatives table? should we force user to add their parents
first? or just ignore them and pretend the new relative has new
parents?

For the purposes of the problem set, you can assume the user would add the parents first. Or you could build your system to allow the user to enter relative without parents and then edit the relative to set the parent id's.
